Welcome to Doctors for Visual Freedom Laser Center and Congratulations! You've taken the first step towards finding your personal best vision. Imagine opening your eyes in the morning and seeing clearly. You look at your alarm clock and can see it! Save time not having to worry about finding your glasses or putting in contacts. I want to remind you that you have an appointment that has been scheduled with our office.

This comprehensive assessment will determine if LASIK or any other Laser procedure is appropriate to you and your eyes. Our surgeon, Dr. Mark Golden will personally perform your examination. Plan to spend 60-90 minutes with us and to have your eyes widely dilated. Your distance vision for driving will be the same, but your near vision will be blurry for several hours. You will also be sensitive to light. We encourage you to bring a friend or family member to your examination as Dr. Golden says, "four ears are better than two." Please bring your glasses, if available. Our office is located at 875 N. Michigan Ave. Suite 1550, inside the John Hancock Center on the Magnificent Mile. There is free parking inside the John Hancock Center for all LASIK patients.

I've attached all the forms that you can fill out in advance then print and bring on the day of your visit.
